Residents and their loved ones are fully supported by an informed and compassionate staff, who specialize in caring for those with physical or cognitive challenges. Freedom Plaza healthcare is Five-Star rated by the Centers for Medicare and Medicaid Services.
WELLNESS CLINIC
Our Wellness Clinic is a great resource for residents. Stop-in for a quick blood pressure check or jump on the scale and check your weight. See us for basic first aid, or feel free to ask a question about medication management - or any other health and wellness topic.
We also schedule monthly wellness education lectures by area professionals.

Custom Care by Freedom Plaza provides private duty services to our residents on short term or long term basis. We are available 24/7 and services can often be started within 24 hours.

REHABILITATION
Rehabilitation at Freedom Plaza is available to residents on an outpatient or inpatient basis, right here on campus. Need some strength training after knee surgery? Balance therapy to prevent a fall? Our fully licensed rehabilitation center, staffed by licensed therapists who specialize in seniors, can provide it.
Our state of the art skilled nursing community provides the equipment, and our therapists provide the expertise. If you need inpatient rehabilitation, we can do that, too.
Rehabilitation Services Include:
- Physical Therapy for Musculoskeletal or Neurological Injuries & Disorders
- Occupational Therapy for Those Struggling with Activities of Daily Living Such as Dressing, Bathing and Self-Feeding
- Speech Therapy and Problems with Swallowing
ASSISTED LIVING
Assisted Living at Freedom Plaza is for those who live independently with assistance as needed for meals, housekeeping, laundry and other daily tasks. Apartments provide privacy and independence, and assistance is available as needed on a daily, or periodic basis.

Skilled Nursing at Freedom Plaza Care Center is available in a special suite here on campus. If illness or injury means that you or your loved one needs specialized nursing care around the clock, our Skilled Nursing Suite is available. Our residents move between levels of care as their needs change. Regardless of the care they need, they are right here at home. Close to loved ones and familiar staff. We accept payment through ALTCS for skilled nursing services.

HOSPICE CARE
